Create a survey

1. Go to Surveys page. 
2. Click +Add a Survey.
3. Add the name of the survey, and click Save.
4. Click + Add a Question.
5. Input a Question, add Help Text if needed, and choose Question Type (Multiple Choices, Checkboxes, Short Answer, Range (1-5) and Range (1-10).
6. After you select the question type for Multiple Choices and Checkboxes, add the values under Add Option, click the checkmark and click Save. For Short Answer, click Save. For Range (1-5) and Range (1-10), you have the option to add High and Low labels, then click Save.
